I have tried to make it easy for you to do this in three ways. First you can download this PDF form. Family Group Sheet (251K) (right click on the link and select 'Save Target As' to save it to your computer) You can use this form two ways. First you can print it out, and make copies to fill out. You will need one for each marriage.

The second way is to fill out the form on your computer. The only problem with this is that you can only print them out. You can not save the filled out form unless you have the full Acrobat program, not just the Acrobat reader. If you need the Acrobat reader program you can find it here: Get Adobe Acrobat

Either way you can mail them to me. I will give you the address upon request.
